  • Overview

    Home Health Nurse

    The staff nurse is responsible for establishing and implementing the care plan for clients receiving Homemaker, Chore and/or Respite services. He/she is responsible for monitoring clients according to program guidelines to ensure quality of care and client satisfaction. The staff nurse will assign and supervise the non-skilled aides.


    Accept referrals in accordance with Agency policy, assesses the client and evaluates their individual needs; prepare chart and implement plan of treatment. The staff nurse acts as a liaison between the physician and other service providers and the patient as well as the Agency. Supervises and teaches aides and participants in in-service training. Reports directly to the Nursing Supervisor and/or Assistant Nursing Supervisor.


    Hospice Staff Nurse

Reports directly to the Nursing Supervisor and/or Assistant Nursing Supervisor.


    Home Health Nurse

    • Schedules work so that all plans of care are implemented within required time frame.
    • Instructs aides to perform specific duties for clients as outlined in the care plan.
    • Maintains an accurate, up to date, chart on all clients in accordance with program guidelines.
    • Supervises services being rendered to the client. Makes supervisory aide visits every 60 days and as needed.
    • Attends all staff meetings as required.
    • Re-evaluates clients as appropriate times to re assess clients needs and make adjustments to the plan of care if needed.
    • Complies monthly programmatic reports as assigned, within the required time frame.
    • Assist in direct service worker training and in-service.
    • Assume staff nurse responsibility in other company programs when needed.
    • Maintains accurate and current clinical and progress notes.
    • Evaluates new referrals as assigned.
    • Obtains verbal orders and start of care from physician.
    • Prepares proper and necessary forms and obtains required signatures.
    • Initiates plan of care in accordance-with the clients needs, physician's orders, and company policy.
    • Provides specialized nursing skills as required.
    • Coordinates services between client, aide and prover of service.
    • Prepares equipment and material for treatment utilizing aseptic technique as required.
    • Assures adequate levels of supplies are maintained.
    • Assist the physician in performing specialized procedures.
    • Initiates appropriate preventive and rehabilitative nursing procedures.
    • Re-evaluates the  client's needs regularly and revises the plan of care according to program guidelines.
    • Informs physician, supervisor, and other personnel of changes in clients condition or needs.
    • Closely supervises nuress' aides by visiting clients residence according-to program guidelines.
    • Teaches and counsels client and family to meet nursing and personal needs.
    • Maintain accurate, complete and current clinical and personnel records in accordance with Medicare, Medicaid, Dept. of Human Services, and company policy.
    • Prepares complete original chart within ninety-six (96) hours of admission and keeps current documentation in order to be prepared or one-site state and federal audits.
    • Observe for problems with aide care and relay those to the supervisor for further guidance and action.
    • Reviews nurse's aide notes, time, travel, and billing forms and ensures they are correct.
    • Prepares written summary of client's condition and sends copy to physician according to program guidelines.
    • Prepares programmatic and other required monthly paperwork in a timely manner in order to meet Agency deadlines.
    • Participates in In-Service programs and teaches other nursing personnel.
    • Assumes on-call duties when assigned.
    • Attends in-service meetings for continuing education.

    Hospice Staff Nurse


    • Evaluates new referrals as assigned.
    • Prepares proper and necessary forms, obtains required signatures.
    • Implements a plan of care.
    • Re-evaluates the client's nursing needs regularly and revises plan of care as needed with Interdisciplinary group approval.
    • Manages care of client in an organized manner and assures that they receive the supplies and equipment appropriate for them.
    • Prepares IDG updates every 14 days or less for IDG meetings.
    • Communicates any problems with the client, family or physician to supervisor.
    • Arrange Crisis care and Respite care.
    • Provides specialized  nursing skills as required.
    • Provides teaching and counseling in the home to patient and family or caregiver.
    • Maintains accurate and current clinical records.
    • Initiates appropriate preventive nursing measures.
    • Coordinates services between client , aide and provider of services.
    • Assists the physician in performing specialized procedures.
    • Prepares equipment and material for treatment utilizing aseptic technique as required.
    • Assures that adequate levels of supplies are maintained.
    • participates in in-service.
    • Maintain accurate medication record.
    • Delivers medicine to patient from pharmacy if needed. Destroys unused controlled drugs.



    • Valid drivers license.
    • CPR certified.
    • Current negative TB skin test and/or state issued health card.
    • Valid Arkansas RN license.
