Intake Coordinator (LPN)

Functions as a liaison to referral sources by responding to the company's centralized referral system and facilitating the provision of home care services. Assist in arranging for uninterrupted continuation of medical care of the clients at home additional services needed for effective care of the client.


  • Develop and maintain a working knowledge of all services and resources provided by the company and available within the community.
  • Collect information from referral sources or potential patients to identify patient needs and determine the company's ability to provide the service(s).
  • Identify and provide alternative community service resources when Area Agency on Aging services are not available to meet the patient's needs.
  • Perform initial telephone screening and advise appropriate staff of patient acceptance.
  • Communicate with referral source to advise of referral status.
  • Identify and verify benefits with payer sources and obtain initial authorizations and frequency of visits. Determine coverage availability for third party non-governmental payers.
  • Work with supervisors, nursing team and fiscal analyst when request for care are inappropriate, incomplete or not covered benefit.
  • Access national or regional account information and communicate to appropriate staff as needed. Work with staff and patients to identify potential solutions as problems are identified with payer sources. Intact with referral sources to facilitate communications to answer questions and resolve problems.
  • Monitor, track and communicate referral sources' satisfaction levels.
  • Establish and maintain a professional relationship with hospital staff, physicians, all referral sources and company staff.


  • Current license as a Licensed Practical Nurse (LPN) in Arkansas.
  • 2 years of home health experience preferred.
  • Knowledge of Medicare, private insurance and Arkansas Medicaid regulations.
  • Excellent written and verbal communication skills.
  • Ability to make appropriate clinical judgement as it relates to the referral process.
  • Proficient computer skills and ability to use and perform key functions of MS Office programs.
  • Valid driver's license.
