Overview

Home Health Nurse

 Accepts referrals in accordance with agency policy, makes home and inpatient visits as assigned and acts as a liaison between physicians and other service providers. Completes necessary paperwork and maintains communication between client, physician, supervisor and staff nurse. Teaches aides and participates in in-service training. Home Health A&B LPN will be supervised by the Nursing supervisor and/or Assistant Nursing Supervisor.

 

Hospice Nurse

Accepts referrals in accordance with Agency policy, makes home and inpatient visits as assigned and acts as a liaison between physicians and other service providers. Teaches aides and participates in in-service training. Report directly to the Nursing Supervisor and/or Assistant Nursing Supervisor.

 

Case Management

The Case Manager will assist the client in obtaining the most appropriate services available and coordinating these services to form a support system which allows a lifestyle with is least restrictive and most desirable to the client. The Case Manager will act as an advocate in representing the citizens of our service area. He or she will be supervised by the Nursing Supervisor and/or Assistant Nursing Supervisor.

 

 

 

 

Responsibilities

Home Health Nurse

 

  • Under supervision of the R.N., carries out the plan of care in accordance with the clients needs,physician's orders and Agency policy.
  • Prepares equipment and material for treatment utilizing aseptic technique as required.
  • Provides specialized nursing skills as required.
  • Assures adequate levels of supplies are maintained.
  • Assists the R.N. and/or physician in performing specialized procedures.
  • Participates in appropriate preventive and rehabilitative nursing procedures.
  • Re-evaluates the client's needs regularly and reports change to the R.N.
  • Coordinates services between client, aide and other service providers.
  • Teaches and counsels client and family to meet nursing and personal needs.
  • Maintains accurate, complete and current clinical and personnel records in accordance with Medicare, Medicade, Dept . of Human Services and company.
  • Assist R.N with preparing written summary of clients condition and sends copy to physician according to program guidelines.
  • Reviews clients plan of care and makes home visits in accordance with plan of care.
  • Informs supervisor, physician and other personnel of changes in clients condition or need.
  • Teaches and counsels client and family to meet nursing and personal needs.
  • Prepares nurses notes, physician orders, lab slips and other documentation as indicated on day of visit.

Hospice Staff Nurse

 

  • Prepare proper and necessary forms, obtaining required signatures.
  • Re-evaluate the clients nursing needs and make recommendations to the R.N. who will revise plan of care as needed with Interdisciplinary group approval.
  • Prepare IDG updates for the meeting every fourteen days or less.
  • Maintain adequate medication documentation.
  • Deliver medicine to the patient from pharmacy if needed.
  • Assume on-call duties when assigned.
  • Supervise oral and I.M. medications taken in home by patient. Provide patient family teaching and medication setup as required.
  • Contact minister or spiritual counseling when patient and/or family request it.

Case Management

  • Prepare monthly billing forms as directed.
  • Keep up to date on the status of each active case until a final disposition is determined.
  • reassess client every six months.
  • Attend meetings as required including litigation proceedings.
  • Act as liaison to other service agencies.
  • Be knowledgeable of current procedures regarding Supplemental Security Income, Medicaid, Medicare, Guardianship, Consumer Protection, Veterans Benefits, public and private pensions, and institutional placement and relocation procedures, whenever abuse, neglect and/or exploitation is found.
  • Assist in the assessments of Adult Protective Services referrals as directed. Accompany Protective Service Consultants to the clients home when necessary.
  • Prepare correspondence as assigned.
  • Coordinate closely with staff nurses to avoid duplication of effort and to more efficiently service clients.

 

 

 

Qualifications

  • Valid drivers license.
  • CPR certified.
  • Current negative TB skin test and/or state issued health card.
  • Valid Arkansas LPN license.
